Com tantas responsabilidades e estresse, Donatella acabou tendo uma recaída com cocaína quando seu irmão Gianni morreu em 1997. Essa tragédia a deixou realmente abalada e, além do seu vício em droga, a personalidade teve um colapso que a forçou a ir até um centro de reabilitação. Tudo isso, aliado aos tratamentos com botox, fizeram com que seu rosto ficasse praticamente irreconhecível e embora seja um ícone da moda, sua aparência não é lá uma referência de beleza.
