Ele interpretou três vezes Barney Ross em The Expendables, quatro vezes John Rambo em Rambo, e sete (sim, sete) Rocky Balboa na franquia Rocky. Mas parece que viver a vida no ringue de boxe causou bastante danos ao rosto de Sylvester. O ator não manteve em segredo que ele teve lesões desde um dedo quebrado até o pescoço fraturado. Parece que, com base em seus filmes recentes, ele tentou consertar alguns de seus ferimentos e os efeitos do envelhecimento com lifting facial, cremes anti-rugas e tratamentos com botox.
